You can either mail or email this information to us in advance of the appointment or you can bring the information with you. As you are probably aware, email communications are not secure and we cannot guarantee the confidentiality of information provided through email.

Where Are We?
We are on the southwest corner of 51st and Yale. We are directly west of LaFortune Park and directly east of Asian Cuisine (formerly Steak and Ale). We are across 51st from the Braum's Store. You may enter our parking lot from 51st or from Yale and you may enter our office from either the east or west side of the building. If you enter on the east side (closest to Yale), go through the lobby to our office door, which is to the right of the elevators. When you enter on the west side (under the red awning), you are in the Therapy 4 Kids end of our office. Go down the hall on your left to the TCCP check-in window.
